We followed exactly the route as layed out in Lonely Planet Trekking in the Central Andes, our hiking Bible. This is the best guidebook.

No one does the hike in the reverse direction because of the logistics of transportation.

Anyone can be unexpectedly stricken with altitude sickness. If it happens to you, head downhill quickly. Even a couple of hundred metres can help.

Keep descending until you feel better. You can tent anywhere.

3-day trek recommended route

... prior to Misti, hike the Colca Canyon from Cabanaconde 3287m for acclimatization. Alternatively, hike Misti immediately after arrival from Cuzco 3326m or Puno which is even higher

night 0 = Arequipa 2230m

... 8AM hired vehicle picks you up at your hotel
... drive to trailhead (2 hours)
... climb to campsite (2 hours)
night 1= Monte Blanco 4800m

... at first light slow, steady climb to the summit (4 hours)
... investigate the summit craters (1 hour)
... easy, sliding descent (2 hours)
night 2 = Monte Blanco 4800m

... a long, easy downhill hike out to Chiguata main plaza (6 hours)
... mini-bus back to Arequipa, likely by mini-bus, US$1, 45 minutes
night 3= Arequipa

A hardy mountaineer might get up at dawn, bag the summit, descend, pack-up camp & still make it out to Chiguata in time to catch daylight transport back to Arequipa.
